During pediatric evaluation, it's essential to create a comfortable environment and build trust with both the child and their caregivers. Begin the assessment by observing the child’s general appearance, posture, activity level, and interaction with others. Use age-appropriate language and behavior to gain the child’s cooperation. Start with non-invasive procedures to minimize fear and gradually proceed to more sensitive areas. Always measure vital signs, including temperature, respiratory rate, heart rate, and blood pressure, adjusting for age-specific norms. Inspect the head for shape, fontanelles, and symmetry.

Examine the eyes for alignment, pupil response, and visual tracking. Check the ears for position, discharge, and hearing response. Evaluate the nose and throat for congestion, inflammation, or abnormalities. Assess the chest for shape, respiratory effort, and auscultate for breath sounds. Evaluate the heart for rhythm, murmurs, and peripheral pulses. Examine the abdomen by inspection, palpation, and auscultation for masses, tenderness, or bowel sounds. Assess the extremities for movement, tone, and reflexes. Review skin for rashes, color, or lesions. Document all findings clearly and share results with caregivers, explaining any necessary follow-up.
